Terms of use

Chart is a personal health record at phr.iamcpdev.me, operated by Chaleb Pommells. It lets its account holder keep their own health information in one place: notes and measurements they enter, readings from their own devices, and copies of records from patient portals they choose to connect. These terms apply to anyone who uses Chart.

Who can use Chart

Chart is a private service. Accounts are created only by the operator, and each account holds one person's records. Providers or caregivers can see information only when the account holder shares it with them.

Connecting patient portals

• You sign in to each portal (for example MyChart) on that portal's own page. Chart never sees or stores your portal password.
• Chart asks only for read access to your own records. It does not change, add to or delete anything in your portal or your provider's system.
• Chart keeps the access the portal grants so it can refresh your records, at most once a day, until you disconnect.
• You can disconnect a portal at any time in Chart under Records, then Connected portals. You can also revoke access from the portal itself.

Not medical advice

Chart stores and displays information. It does not diagnose, treat or give medical advice, and it is not a substitute for your care team. Records copied from portals may be incomplete or out of date; your provider's records are the official source. In an emergency, call 911.

Your responsibilities

• Use Chart only for your own records, or for records you are legally allowed to manage.
• Keep the devices that hold your passkeys secure, and tell the operator promptly if you think someone else has accessed your account.
• Check information before relying on it or sending it to someone else, including by fax.

Availability

Chart is provided as is. The operator works to keep it available and secure but does not promise it will be free of interruptions or errors, and is not responsible for the availability of third-party portals, fax carriers or hosting services.

Ending use

You can stop using Chart at any time and ask for your account and records to be deleted. How information is handled is described in the privacy policy.

Changes

If these terms change, the effective date above will change, and continuing to use Chart means accepting the updated terms.

These terms are governed by the laws of the State of Texas.
